Social media management for rehab centers: what's included and what it costs

By FirstCall Social · October 12, 2026 · 7 min read

Rehab social media management means an outside team plans, writes, designs and posts your center's content every week, then watches the accounts, so your staff don't have to. For a treatment center it should also include a privacy review of every post, because one wrong photo or reply can expose a patient. Done-for-you organic management from FirstCall starts at a flat $997 a month. The real question is what it replaces: hours of your team's time, or accounts that sit quiet.

Five carousel slides for Fernvale Hollow Recovery, a sample center (fictional), fanned out: photo-led posts about the 24/7 admissions line, what happens when you call, the rooms and insurance, each with the center's logo, town and phone number.
A week of posts built from the center’s own facts, photos and city. Sample center (fictional); photos are stand-ins. See the full example week.

This guide is written for owners, CEOs and marketing directors deciding whether to keep social in-house, hire someone, or hand it off.

What rehab social media management includes

"Management" means different things to different vendors. Some only schedule posts you write. Others do everything. Before you compare prices, know which of these jobs you are paying for.

Job What it means for a treatment center Done-for-you service?
Strategy Which platforms, which audiences (people seeking help, families, referents), what each week covers Yes
Content calendar A planned week of posts, set in advance, around your programs and team Yes
Writing Captions in plain, hopeful, accurate language with no outcome promises Yes
Design and video Graphics, short clips, photo edits that fit your brand Yes
Posting Publishing on schedule across every account Yes
Privacy review Checking each post for patient faces, names, details and testimonials before it goes live Should be yes. Ask.
Community watch Flagging comments and messages so your team can respond safely Usually
Reporting Reach, views and activity, monthly Yes

If a vendor's proposal doesn't list the privacy review, assume it isn't there.

Your three options, side by side

Most centers land on one of three setups. None is free. The cost is either cash, staff time, or both.

Option What you pay Your staff time Main risk
Staff member "does social" on the side No new cash Several hours a week, pulled from admissions, ops or clinical work Posting stops the week things get busy
Hire an in-house social media manager Salary, benefits, design and scheduling tools Hiring, training, managing and reviewing One person to cover strategy, writing, design and video, and a gap if they leave
Done-for-you treatment center social media management A monthly fee Little once onboarding is done Choosing a vendor who doesn't know behavioral health privacy rules

The side-of-desk option looks cheapest and is often the most expensive. The person doing it usually has a more urgent job, and an account that posts twice and goes silent for a month tells families nobody is minding the store.

What rehab social media management costs

You will see three common pricing models:

  • Hourly or per-post. Common with freelancers. Cheap per unit, but you manage the work and the volume tends to drift.
  • Flat monthly fee. A set number of posts and services each month. The easiest to budget and compare.
  • Retainer plus ad spend. Management fee for organic posts, plus a separate budget if you also run paid ads. Keep the two lines apart when you compare quotes.

When you compare, put every quote on the same footing: how many posts a week, which platforms, whether video is included, who writes the captions, and how much of your team's time it needs.

One model to refuse outright: paying a marketer per call, per lead or per admission. The Eliminating Kickbacks in Recovery Act (EKRA) makes it a federal crime to pay or receive remuneration for referring patients to a recovery home or clinical treatment facility, with penalties of up to $200,000 and 10 years in prison per occurrence (18 U.S.C. ยง 220). A flat fee for content is a different thing from a fee tied to patients. Talk to your healthcare attorney about any marketing contract, and see our EKRA guide.

Privacy: the part generic agencies get wrong

A social media manager for a coffee shop can post customer photos and reply to every review. A treatment center can't. Two rules shape what you can post:

  • HIPAA protects patient health information, including the fact that someone was your patient. HHS's Office for Civil Rights reached a $30,000 settlement with a New Jersey mental health practice that disclosed patients' information while replying to negative online reviews (HHS).
  • 42 CFR Part 2 restricts how federally assisted substance use disorder programs use and disclose patient records, and most disclosures need written consent (eCFR).

In practice that means no patient faces in the background of a group-room photo, no "glad you're doing well" replies to a former client's comment, and no testimonials without a process your compliance lead signs off on. Ask any vendor who checks each post for these, and when. Our compliance page explains how we handle it.

How to choose a rehab social media agency

Use this checklist on every call.

Ask A good answer sounds like
How many posts a week, on which platforms? A specific number, in writing
Who reviews posts for patient privacy? A named step before anything goes live
How much of our time does this take? A specific estimate for onboarding and each month
How are you paid? A flat fee. Never per lead, call or admission
Who on your team will do the work? Named people and their roles
How long until our first posts are live? A date, not "it depends"
Who checks content before it posts? A trained reviewer, with a second check for sensitive topics

If you're still deciding how much volume you need, our guide on how often a rehab should post on social media lays out an example week.

The only results we share are from our founder's own treatment program: 844 accounts reached in 5 days, up from 8, and one TikTok post passed 9,145 views. Individual results vary. That's reach, not a promise of calls or admissions. Not a promise of results. See full pricing and how it works.

Frequently asked questions

How much does social media management cost for a rehab center? It depends on volume, platforms and whether video and privacy review are included. FirstCall's flat monthly fee starts at $997 a month. Whatever you're quoted, compare posts per week and staff time, not just the monthly number.

What does a social media manager do for a treatment center? They plan, write, design and post content, watch comments and messages, and report on reach. For a treatment center they should also screen every post for patient privacy before it goes live.

Is it legal to pay a social media agency per admission? Paying for patient referrals can violate EKRA, a federal law covering recovery homes and clinical treatment facilities. A flat fee for content is the safer structure. Ask your healthcare attorney about any contract.

Can we post patient testimonials? Only with a consent process your compliance lead approves, because HIPAA and 42 CFR Part 2 both apply. Many centers skip patient testimonials and feature staff, programs and the facility instead.

How long does it take to get started? With FirstCall, your feed goes live within 10 business days of your kickoff call and completed onboarding form, with about 2 hours of your team's time in the first week.
